Claudio Ranieri makes his ‘debut’ on the Roma bench with Artem Dovbyk upfront, as leaders Napoli have Scott McTominay and Romelu Lukaku.
It kicks off at the Stadio Diego Armando Maradona at 17.00 GMT.
Roma are on their fourth different coach of 2024, having sacked Jose Mourinho in January, Daniele De Rossi in September and Ivan Juric under two months later.
Ranieri has come out of retirement to help his beloved Giallorossi on this third stint at the club.
There are some problems to deal with, as Paulo Dybala is not fit to start, with Mats Hummels also weakened by flu symptoms.
Mario Hermoso, Eldor Shomurodov and Alexis Saelemaekers did not make the trip, so Dovbyk is supported by Stephan El Shaarawy and Lorenzo Pellegrini.
Napoli are feeling the pressure at the top of the table, with their lead whittled down to just one point going into this weekend after defeat to Atalanta and the draw with Inter.
McTominay had a scare for Scotland, but it proved to just be a bruise to the left foot and he is passed fit to start with Lukaku and Khvicha Kvaratskhelia.
Matteo Politano is again preferred to David Neres in the trident attack, but after 51 days Stanislav Lobotka returns to the starting XI.
